Birth Certificates

The Navajo County Health Department is able to provide certified copies of birth certificates for persons born from 1950 until present. In most situations we can provide same day service. The charge for a certified copy of a birth certificate is $10.00, payable by money order, cashier’s check, cash or personal check. Please note that when paying by check it will delay receipt of your certificate of up to three weeks.

For home birth registrations, or to file an affidavit of paternity or acknowledgement of paternity an appointment is required.

The documentation needed to obtain a certified copy of a birth certificate is:

  • Application Request Form - Available here or in the office.
  • A government issued picture ID, i.e. a valid driver’s license, state ID etc.
  • If not for self, documentation showing your relationship to the person the requested certificate is for.
  • When mailing in a request the application for a birth certificate must be notarized and accompanied by a valid government issued ID.

For birth certificates prior to 1950, you must apply directly to the Arizona Department of Health Services, Office of Vital Records.

Death Certificates

The Navajo County Health Department provides certified copies of death certificates within 30 days of registration. We cannot process death certificates after the 30 day registration time period has passed. The charge is $10.00 per certified copy, payable by money order, cashier’s check, or cash only. Personal checks are not accepted for this service.

The documentation needed to obtain a certified copy of a death certificate is:

  • Application Request Form - Available here or in the office.
  • A government issued picture ID of the person requesting the certificate, i.e. a valid driver’s license, state ID etc.
  • Documentation showing the requestor’s relationship to the decedent.
