Fatality Reviews


To reduce preventable deaths through systematic, multidisciplinary, and multimodality review of fatalities of Navajo County residents.

Fatality reviews are conducted at the county level to identify risk factors contributing to preventable deaths of community members. A multidisciplinary team examines records relating to the decedent to better understand how and why the death occurred. Recommendations for system changes and prevention efforts, including education, training, policies and procedures, and legislation, are generated during the review process to prevent future deaths.

Review teams consist of representatives from:

  • public health
  • public safety
  • medical and behavioral health
  • courts
  • tribal organizations
  • people with lived experience
