How to become a WIC client?

  1. Enter your information in the Participant Portal and someone will call you to make an appointment

  2. Bring proof of your household income to your appointment (including but not limited to):
    • 30 days of recent paystubs, AHCCCS Letter, SNAP Letter, tax return, Social Security statement, public assistance or unemployment compensation award letter, or other statement of income in the household.
  3. Bring proof of residency stating your current physical address (including but not limited to):
    • Current Utility bill
    • Current cable bill
    • Current rent receipt
    • Current mortgage receipt
  4. Proof of Identity (including but not limited to):
    • Infants or Children: birth certificate, crib card or tribal enrollment card, immunization record, etc.
    • Adults: driver's license, state ID card or tribal enrollment card, birth certificate, passport, immigration or naturalization record, etc.
  5. Everyone who will be enrolling into the WIC Program must be present at the certification appointment.
  6. We now offer virtual and phone appointments for most services, ask a WIC team member!
